To ensure accurate information about facility incidents staff must use the most appropriate form and/ or use more than one form if need-be when writing or creating a report – about any incident.
Generally:
- Incident Form is used for First Aid responses
- Person Description Form is used when you need to make detailed notes regarding any interactions with another person.
- Additional Information Form is used when you need to write more complex details and/ or provide extra information.
